WAIVER:  By signing up for a class, I acknowledge that I am voluntarily participating in Zumba Gold classes with Lesley. I understand that physical exercise involves inherent risks and hazards, including but not limited to, injuries and falls. I agree to assume full responsibility for any risks, injuries, or damages, which I might incur as a result of participating in the classes.

I hereby release and discharge Zumba Gold with Lesley and any associates from any and all claims or causes of action, and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action against Lesley for injury or damages that I may sustain as a result of participating in the program.
